Listed on the organization's own publication of this base URL, not on a retrieved conformance document: base URL printed verbatim on CHRISTUS Health Plan's own site, under the literal heading 'Base FHIR Provider Directory URL', on a page the plan's interoperability section links to (page footer: last updated 2026-06-24). The host is the vendor's; attribution to CHRISTUS rests on the plan printing this exact string on its own domain. The plan publishes no Patient Access base URL: that section names a different vendor and links off-site to the vendor's documentation (recorded 2026-08-19). Published at https://www.christushealthplan.org/privacy-and-patient-rights/interoperability/healthsparq-fhir-api. On the verification date this probe observed: HTTP 404 to a GET of /metadata. The same page describes a 'public token' flow for this directory - POST an insurer code and brand code to a public token URL, then pass the result as a Subject-Token header - so the surface CMS requires to be reachable without authentication is reachable only after a token exchange, and its conformance document is not reachable at all from here.
