The base is the server URL plus the /pd/R4/metadata route printed together in the OpenAPI spec the portal embeds for its 'CMS Interoperability Provider Directory Metadata' product. Two things are recorded rather than repaired: the document this route serves is the same clinical-resource CapabilityStatement as the Patient Access route, not a Plan-Net one, and the resource base the portal documents for the directory itself - which its product page calls an open access service that does not require registration - answers 404 to a GET of /metadata (candidate log).
